Helpful Information
Prescription Policy:
We will fax all new prescriptions and refills to your pharmacy. If this is a new prescription, please allow enough time for the pharmacy to process it.
Refills:
We ask that you contact your pharmacy 48-72 hours before you need your medication refilled, even if you have no refills left. The pharmacy will fax us the request for your medication, and we will contact them directly to refill your medication.
When the refill request is received, Dr. Merola will determine if a follow-up appointment will be needed before the request can be completed. It is very important that you not wait until you are out of medication to contact the pharmacy.
We do not process prescription refill requests on weekends or after hours. Lost, misplaced or stolen pain medications cannot be refilled.
Mail Order Refills:
If you use a mail order pharmacy service, please leave a message with the front desk staff, and someone will contact you to complete your request.
Triplicate Prescriptions:
Please call 3-5 days ahead of time for triplicate prescriptions. There is a small fee per triplicate refilled, and they must be picked up at the office, or mailed to your home.
Cancellation Policy:
We do our best to see each patient in a timely manner when they arrive. We do not double book appointments so you have time to address your needs with your physician. We ask for mutual respect for each others time commitments.
If you call and cancel your appointment with less than 24 hours notice, it is extremely difficult to fill that appointment time with another patient. Letting us know ahead of time benefits us all by allowing that time to open for other patients who require our services.
There is a $25.00 fee for missed appointments, and appointments canceled without 24 hour notice. Insurance does not cover this charge. We understand that sometimes last minute cancellations are unavoidable, but please be considerate and give adequate notice.
Annual Visits and Sports Physicals
An annual visit is a marvelous way for the patient and doctor to establish a plan for future wellness. It is a chance got the patient to focus on general health issues, risk screening, labs, vaccinations, and to establish a complete health history. If you have any chronic or acute health issues, a separate visit may be required to address these.
Important things to note:
- Most insurance companies will only pay for 1 full physical per year. If you schedule before 12 months has passed, your insurance will not pay.
- School sports, band, Boy Scouts, over seas travel and other athletic events usually require a yearly exam. Please note the deadline for the date they must be completed. Waiting until late summer or the start of school may make it difficult to schedule in a timely manner.
- Please bring any required forms with you for your physical. Prior to your visit please read over and fill out any patient information before you arrive.
- Please bring current immunization records for all minors.
